A birth weight greater than the 90th percentile for gestational age or birth weight 4000-4499 g at term regardless of period of gestation.
Coding Note
When both birth weight and gestational age are available, priority of assignment should be given to birth weight.
Exclusions
- Syndrome of infant of mother with gestational diabetes
- Syndrome of infant of a diabetic mother, type 1 or 2, nongestational, insulin dependent
